Why Diabetes Hits Korean Families Harder Than Many Expect

Type 2 diabetes is often framed as a disease of obesity, but the pattern among Koreans and Korean Americans tells a more complicated story. In South Korea, roughly one in seven adults lives with diabetes, and the Korean Diabetes Association has reported that close to a third of adults have prediabetes. A large share of those people are not overweight by Western body-mass index standards, which is exactly why the condition gets missed.

Korean bodies tend to store fat around the organs and inside the abdomen before it shows up as visible weight gain. Researchers call this a “thin-fat” pattern, and it drives insulin resistance at lower BMIs. A 45-year-old woman in Los Angeles who weighs 128 pounds and has a normal BMI can still have an A1C of 6.8. Her doctor may not think to test her. That gap between appearance and risk is the core problem.

Migration makes things worse. Korean Americans who grew up in the United States eat more processed food and drive more than relatives in Seoul, while still keeping a rice-centered diet. Studies of Korean immigrants consistently show higher diabetes rates than in the home country, with risk climbing the longer a family has been here. Genetic variants common in East Asian populations, including one that affects how the pancreas develops and functions, add another layer that has no equivalent in most European-ancestry patients.

The Signs That Show Up in Feet and Nails First

Peripheral neuropathy is the complication Korean patients mention most often, and it usually announces itself in the toes before anywhere else. Early signs of diabetes in the feet include numbness that feels like wearing a sock when you are barefoot, burning at night, or a pins-and-needles sensation that comes and goes. Because the loss of feeling is gradual, many people ignore it until a small blister turns into an ulcer.

Check your feet after every shower. Look at the tops, the soles, and between the toes. If a cut or callus takes more than a week to heal, that is a reason to call the clinic, not to wait. In Korean immigrant households, where many people work standing jobs in restaurants, dry cleaners, and nail salons, foot problems get dismissed as part of the job. They are often the first clinical clue.

What your toenails can tell you

Diabetes nails are a real phenomenon, though they are not a diagnosis on their own. Poor circulation and slower healing make fungal infections more common and more stubborn. Thickened, yellowed, crumbly nails that keep returning after treatment, or repeated ingrown toenails, are worth a blood glucose check. So are recurring skin infections on the lower legs. Medicare covers a foot exam once a year for people with diabetes, and most commercial plans cover it too, yet it is one of the most skipped appointments on the schedule.

Rice, Alcohol, and the Meals That Raise A1C

You do not have to give up rice, but volume and timing matter. A typical Korean home meal can put two or three cups of cooked white rice on one plate, which is 90 to 130 grams of carbohydrate before the banchan are counted. Swapping half the white rice for barley or brown rice, and eating vegetables and protein first, blunts the glucose spike measurably. Many Korean Americans find that a continuous glucose monitor makes this concrete: they can see what a bowl of rice does to them personally, which is more convincing than a handout.

Screening is the part families control. The American Diabetes Association recommends testing from age 35 for everyone, but Asian American adults should ask earlier, especially with a family history or a waist measurement over 35 inches in women and 40 in men. A fasting glucose test costs under $30 at most labs, and an A1C runs $40 to $70 without insurance. Korean-language clinics and community health centers in Los Angeles’s Koreatown, Flushing, and the Seattle suburbs offer these tests on sliding scales.

One practical warning: several over-the-counter cold and cough syrups, including some versions of Nyquil, contain high-fructose corn syrup or significant alcohol. Read the label if you are managing blood sugar. Sugar-free formulations exist and work just as well.

What to Ask at Your Next Appointment

Bring three questions. First, what is my A1C and what is my target? Second, when did I last have a dilated eye exam and a foot check? Third, does my kidney function need testing with a urine albumin-to-creatinine ratio, since diabetic kidney disease progresses faster in Asian populations than many clinicians assume. Diabetes management technology has improved sharply; a CGM and an insulin pen with a memory function turn guesswork into data, and most insurance plans now cover both for people on insulin.

Language matters more than people admit. If a parent understands medical instructions better in Korean than in English, ask the clinic for a professional interpreter rather than relying on a child to translate. Errors in medication dosing are common when a family member improvises. Community organizations and Korean-language health lines exist in most metropolitan areas, and using them is not an admission of weakness. It is simply how you get the right information into the right hands.
